	Select this option if you would like to start a NEW team. If you start a team, you will automatically be designated as the team captain. If you are a Survivor AND want to start a team, make sure you select "I Have/Had Cancer" in Question #1 to be marked as a Survivor.
	Select this option if you would like to join an existing team! If you are a Survivor AND want to join a team, make sure you select "I Have/Had Cancer" in Question #1 to be marked as a Survivor.
	Select this option to register for this year's Survivor Lap and not join a team! I am a survivor who is NOT a team captain or team member, but I would like to walk in the survivor lap. Please note, this option will not allow you to start or join a team.
	Select this option if you would like to participate but are not a Survivor and do not want to be on any team. If you would like to be placed on a team, please contact your local Volunteer Online Chair, Alexis Maliga at alexis.maliga@cancer.org, after signing up as an individual.
