Below are the Youth Agreement Forms and Chaperone Agreement Forms. All 18&U participants must have a signed youth waiver to participate. Also, for every 10 youth on a team, you must have one chaperone (please have your chaperone fill out the agreement form).
DHS Students: please return your signed youth & chaperone forms to the folder on Room A310's door by Wednesday, May 15th!!

**Remember: in order to be granted access to the event, your chaperone must be on site. Also, to be given an overnight wristband, you must be pre-registered and have turned in your Youth and Chaperone Forms!!
The journey to end cancer starts with a single step. The American Cancer Society invites you to take that step with us by joining the global Relay For Life movement. When you walk to end cancer at a Relay event, it's your opportunity to not only honor cancer survivors and remember loved ones lost, but also to raise awareness about what we can do to stay well from cancer and raise money to help fuel the world's largest walk to end cancer.

THIS YEAR, selected Relay event(s) in our community are offering the unique opportunity to participate first-hand in a landmark new study, the Cancer Prevention Study-3 (CPS-3).The American Cancer Society will be enrolling people into CPS-3, a long-term study that will help researchers better identify the lifestyle, environmental, and genetic factors that cause or prevent cancer. Please spread the word and visit the Cancer Prevention Study-3 website to find an enrollment location near you and for more information about how to participate in research today for a cancer free tomorrow!
