We welcome the exuberance, creativity, and idealism young people bring to Relay For Life. The participants in our high school and college events continue to impress us with their maturity, sensitivity and, perhaps most of all, their commitment to rid the world of cancer.
We hope you'll have a great time as you raise money and help fight cancer. Our goal is to provide a safe, fun, and positive experience for all Relay participants, as well as all survivors and caregivers who come as guests.
In order to do that, a few rules are necessary and two forms must be signed - one by the youth participant and a parent, and the other by their chaperone at the event.
We require all Relay participants under the age of 18 to fill in the Youth Participant Form and have it signed by a parent or guardian. It can be turned in at a team captains' meeting or mailed to your local American Cancer Society office. We require one chaperone over the age of 25 for every 5 youth team members.

And, chaperones, who must be 25 years of age or older, are required to fill out the form below and bring it to a team captains' meeting or mail it to their local American Cancer Society office.
