Youth Participation Forms

Relay For Life is a family-friendly event, and to keep the experience safe and rewarding for all, participants aged 18 years or younger must have parental consent to participate in Relay For Life. A Chaperone must be present for all Youth Participants for the entire event. Please complete the Youth Participation and Chaperone Forms below and return them to your local American Cancer Society Office.
